Thank you for choosing MSPG as your healthcare provider.  We are committed to providing patients with quality and affordable care. 

 

Below are some highlights from the MSPG financial policy:

 

  • All patients must complete a New Patient Registration Form. This will be entered into our practice management system to maintain accurate information for proper billing.  If you fail to provide us with the correct insurance information, or your insurance changes and you fail to notify us in a timely manner you may be responsible for the balance of a claim.  

 

  • We submit patient claims and assist in any way we can to help get claims paid.  Your insurance company may not accept information from our office and may need you to contact them directly.  It is your responsibility to comply with their request.  

 

  • It is our policy at MSPG to charge $25 for missed appointments that are not cancelled within a reasonable amount of time.  These charges are the patient's responsibility and billed directly to the patient.  Please help us serve you better by keeping your regularly schedule appointment.

 

  • All co-payments are due at the time of service.

 

  • For patients that are uninsured or that are unable to pay at the time of service, please contact our billing department to set up an agreed upon payment arrangement.
