Below is a listing of items you will need to bring to each visit:

  • A Photo ID, State Issued

  • Primary & Secondary Insurance Cards

  • Copay or non-insurance fees

  • Medical Records from other facilities treating the same issue (first visit only)

  • A smile!  That's right we said it.  In order to make your records more secure we take photos of all patients for their medical chart.

 

If you give us a valid email we will be able to send you an invite to view your electronic medical records online.

 

We strive to provide a quick and efficient check in process.  Being prepared helps us elimate wait times.
